Our Patient Care Coordinators
As a Patient Centered Medical Home, McCook Clinic employs Patient Care Coordinators to help our patients stay healthy. Patient Care Coordinators help to ensure you are scheduled for important tests and screenings, answer questions about medicines you are taking and relay important information back to your doctor. One of our Patient Care Coordinators may call you after or between visits to remind you about tests and screenings. You may also hear from them following a stay in the hospital or a visit to the Emergency Room. They are here for you!
Lorie Berry, LPN - Care Coordinator
Lorie joined the McCook Clinic in 2003. Before transitioning to the care coordination team, she worked for many years on the nursing floor. These years of hands-on nursing experience gives her a better understanding of our patients needs. Lorie's role on the care coordination team focuses mainly on transitioning our patient's care back to the clinic from recent ER visits and hospital admissions.
